Bethel Web Forms Access Point

The following list shows all of the forms currently (October, 2017) in use on the HostGator hosted web site. This will (hopefully) provide a linking facility for using these forms on the new 360unite hosted site.

• Visitor Survey -This is the form that Bethel visitors can use to provide feedback information about their experience. This form is set to send email to kherwald@blcbcs.org with a BCC: to webmaster@blcbcs.org. Access this form as: http://forms.blcbcs.com/v_survey3.php
   
• Facility Use Request - This is the form from the Resources page to request use of rooms in the building and/or to request an event be added to the church calendar. This form is set to send email to facilities@blcbcs.org (Nic Duncan) with a CC: to kherwald@blcbcs.org and a BCC: to webmaster@blcbcs.org. In addition, a copy is sent to AV-Admin@blcbcs.org if Audio/Video support is requested, and a copy to kbentley@blcbcs.org if child care is needed. • Member Facility Use Request - This is the form from the Resources page to request use of rooms in the building by church members for none church-related events. This form is set to send email to facilities@blcbcs.org (Nic Duncan) with a CC: to kherwald@blcbcs.org and a BCC: to webmaster@blcbcs.org. In addition, a copy is sent to AV-Admin@blcbcs.org if Audio/Video support is requested, and a copy to kbentley@blcbcs.org if child care is needed. • Contact Us - This is the Contact information form and an option to request information or make suggestions. This form is set to send email to office@blcbcs.org and a BCC: to webmaster@blcbcs.org. Access this form as: http://forms.blcbcs.com/contacts.php
   
• Add to E-memo Request - This is the form to request being added to the e-memo mailing list. This form is set to send email to office@blcbcs.org and a BCC: to webmaster@blcbcs.org. Access this form as: http://forms.blcbcs.com/enews.php
   
• Living With Grief - This is the registration form for Pat Wallace's 'Living With Grief' program. This form is set to send email to pat.wallace@juno.com(Pat Wallace) and office@blcbcs.org with a BCC: to webmaster@blcbcs.org. Perhaps this would link in-line using an iframe. Access this form as: http://forms.blcbcs.com/grief.php
   
• Ornament Sales This was a 'quicky' form put together for the youth group ornament sales. Obsolete now, it may be a useful template for a new form since it is short but has the essential elements.Access this form as: http://forms.blcbcs.com/ornaments.php
